Treatment of Open Fractures.

چکیده
While disease has varied in character with differing civilisations and the circumstances of living, injuries have varied only in extent and the methods of infliction; the problem of their treatment has remained the same. The treatment of an open wound involving injury to bone -surely one of the most primitive of injuries-always has and always will include treatment of the wound, of the part injured, and of the person; the essential demands of treatment have never altered, but only during the last 25 to 30 years has it been possible to organise these on an appropriate basis. A very brief review of the history of treatment, confined to this period, is not only instructive bitt imperative to a proper perspective of the subject; it covers the brief interval since the days when amputation, immediate or remote, was the usual ending of an open fracture, and during which certain principles of treatment have been more surely recognised and insisted on as a basis of treatment. Within the framework of these principles there are variations of opinion on the actual practice, on the details of technique, but such differences may be regarded as competitive in an effort to fulfil more adequately the requirements of the principle. For example, the different methods of splinting are matters of technical detail, the principle involved is immobilisation of a part injured; again excision of a recent wound is a principle of treatment, its extent is still a matter of opinion. The treatment of open fractures can be conveniently brought under two headings for the purposes of discussion.

Treatment of Open Tibial Fractures: Convert-ing or Continuing External Fixation?
Background: The treatment of open tibial fractures is still an orthopaedic challenge and full of complications. In many cases the use of external fixator that has been known as a non-union machine is obligatory with a high incidence of pin track infection and other complications. The aim of this study was to compare the use of external fixation as a definite method for treatment of open tibial ...

Background: In children, inappropriate treatment of open femoral fractures may induce several complications. A few studies have compared the external fixator with flexible intramedullary nails in high-grade open femoral fractures of children. The present study aims at comparing results of these two treatment methods in open femoral fractures. Methods: In this descriptive analytical study, 27 ...
متن کاملThe Best Option in Treatment of Modified Mason Type III Radial Head Fractures: Open Reduction and Internal Fixation Versus Radial Head Excision
Background: Radial head fractures commonly occur during elbow traumas. Among those, treatment of Mason type IIIfractures is still under controversy. Common treatment methods for these fractures include open reduction and internalfixation (ORIF) as well as radial head excision. In this study, we compared long-term outcomes of both methods intreatment of patients with Mason type III fractures of ...

Introdouction: The incidence and etiology of maxillofacial fractures are influenced by social, cultural, and environmental factors. Understanding the cause, severity, and temporal distribution of maxillofacial trauma provides an insight into the behavioral patterns of people from different countries and can also assist in establishing clinical and research priorities for effective treatment and...
متن کاملLong-term Results, Functional Outcomes and Complications after Open Reduction and Internal Fixation of Neglected and Displaced Greater Tuberosity of Humerus Fractures
Background: Humerus fractures include 5% to 8% of total fractures. Non-union and delayed union of GT (GT) fractures is uncommon; however they present a challenge to the orthopedic surgeons. Significant controversy surrounds optimal treatment of neglected fractures. The purpose of this article was to perform a comparative study to evaluate the outcomes of open reduction and internal fixatio...
متن کاملThe functional outcome of surgically treated unstable pelvic ring fractures by open reduction, internal fixation
Abstract Background: Unstable Pelvic fracture, a result of high energy antero-posterior compression injury, has been managed based on internal fixation and open reduction. The mode of fixation in Unstable Pelvic fracture has, however, been a subject of controversy and some authors have proposed a need to address the issue of partial breach of the pelvic ring elements in these injuries. This st...
